Service Availability

Due to high demand for our quality services, some programs may have waiting lists. We work diligently to match individuals with appropriate services as quickly as possible and will keep you informed throughout the process. Individuals added to the wait list will be notified upon being added and contacted by RCO upon admission to the program of their choice.

Service Transition and Exit Criteria Statement:

Individuals receiving services through Rock County Opportunities’ DHS 245D licensed programs may transition to different service levels or exit services when they have achieved their individualized goals, no longer require the current level of support, or when circumstances change that affect their service needs.

All service transitions and exits are planned collaboratively with the individual, their support team, and case manager to ensure continuity of care and successful outcomes.

Support Team Notification Statement:

Your support team and decision-making entity at Rock County Opportunities includes, but is not limited to, the Executive Director, Program Director, Program Manager, Employment Specialist, Designated Coordinator, and Direct Support Professionals who work together to provide coordinated services and support.

This team collaborates to ensure you receive comprehensive, individualized services that meet your needs and help you achieve your goals.

Rates and Payment Options

RCO is a Licensed 245D Provider. RCO programs and services can be funded through the Elderly Waiver (EW), Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), Developmental Disabilities (DD), and Consumer-Directed Community Supports (CDCS) Waivers.

The main payer source for Minnesota DHS Home and Community-Based Services (HCBS) is Minnesota Health Care Programs (MHCP), particularly through the state’s Medicaid HCBS waivers and the Alternative Care (AC) program.

Rates are set by the State & are not controlled by the Case Manager or RCO. Rates can increase or decrease based on the State Legislature.

Other payer sources can include Private Pay, Support Grants and Contracted Services
